Requesting a Verification of Your Missouri Licensure Be Sent to Another State

If you are applying for licensure in another state, you will likely be required to submit verification of your Missouri license to the state board in which you are applying. Verifications of licensure include the licensee name, license number, license status, original issue date, expiration date, and disciplinary action, if applicable. The verification letters also include an official seal and signature. There is a $25.00 fee for this service.

Please include your name, address, license number, telephone number, and the name and address or e-mail address of the state board where you want the verification sent.

Requesting Another State Send Verification of Licensure to Missouri

If you are applying for licensure in Missouri, you are required to submit verification of licensure from any other state where you are currently licensed or have ever been licensed. The other states of licensure may submit this verification on their own form or on Missouri's Form 375-0335. The verification of licensure must be received directly from the other state's licensing agency, but can be sent via mail or e-mail:
