Change to Verification of Licensure

As of October 26th, 2018, the Missouri State Board of Registration for the Healing Arts will only send licensure verifications to other state boards. In the future, please utilize the Licensee Search function or the Non-Active listing on our website for primary source verification.

Verification of Licensure/Examination Scores

VERIFICATION OF LICENSURE

The Missouri Board of Healing Arts does not charge a fee to verify a license to another state or agency. Please email your request to HAcompliance@pr.mo.gov or fax (573) 751-3166 attention Verifications. You can also mail your request to P.O. Box 4, Jefferson City, MO 65102.

Please include the following:

• The full name of the licensee
• License Number
• Profession
• Where the Verification needs to be sent

EXAMINATION SCORES

The Missouri Board of Healing Arts charges a fee to provide examination scores to another State Board if you took the Missouri State Board examination (Not FLEX or USMLE). Please submit the fee along with the following information to the Board of Healing Arts, P.O. Box 4, Jefferson City, MO 65102:

• The full name of the licensee;
• Profession;
• License number;
• Approximate date of examination;
• Address that the scores need to be sent.

The fee for physician examination scores is $25.
The fee for physical therapist examination scores is $25.
