EXTENSION OF PROVISIONAL LICENSE

To request an extension of a provisional license, please submit a written request, signed by both the supervisee and the supervisor, stating why the extension is needed. The request can be emailed to profcounselor@pr.mo.gov as a PDF document. Or, you can mail your request to Missouri Committee for Professional Counselors, PO Box 1335, Jefferson City MO 65102. There is no cost.
