Board Member Appointment Application

On August 28, 2018, Senate Bill 843 took effect which gave the Director of the Missouri Division of Professional Registration the authority to appoint members to the following boards: Missouri Acupuncturist Advisory Committee, Anesthesiologist Assistants Advisory Committee, Advisory Commission for Dental Hygienists, Interior Design Council, Missouri Advisory Commission for Clinical Perfusionists, Advisory Commission for Physical Therapists, and Missouri Advisory Commission for Physician Assistants.

To apply to be a member on one of the boards or commissions listed above, complete the Board Member Appointment Application form. You can mail, fax, or email the completed form to the contact information on the application.

Board Members to any board or commission not listed above are appointed by the Governor of the State of Missouri. Apply to these boards or commissions at https://boards.mo.gov/UserPages/Home.aspx.
