Below are the necessary forms for filing a complaint with the State Committee of Psychologists. The complaint form will provide the committee with specific details regarding your complaint and the consent form will authorize the release of confidential information. Please note that the release form must be notarized.
Upon receipt of your written complaint, the office of the State Committee of Psychologists will begin the Public Complaint Handling and Disposition Process as set forth in 20 CSR 2235-4.030 of the Psychology Practice Act and Rules.
