Inactive Physical Therapist License

If you would like to place your license in an inactive status, please complete the Inactive Licensure Status form. To have your license switched back to active status, you will need to complete the Inactive Reinstatement Application. If your license has lapsed due to non-renewal, a different application needs to be completed.
