LPN Renewal Information
Current LPN licenses expire May 31, 2006. Please check your LPN license. If you are practicing nursing in the State of Missouri and your license does not say "valid through May 31, 2006" you may be practicing nursing in Missouri without a current license. Please contact the Board office at 573-751-0681 as soon as possible for additional renewal information. The form to renew an expired Missouri LPN license can be found by clicking on the Licensure Info/Forms tab.
RN Renewal Information
Please check your RN license. If you are practicing nursing in the state of Missouri and if your license does not say "valid through 04/30/07" - you may be practicing nursing in Missouri without a current license. Please contact the Board office at 573-751-0681 as soon as possible for additional renewal information. The form to renew an expired Missouri RN license can be found by clicking on the Licensure Info/Forms tab.
ADDITIONAL INFORMATION FOR RNs AND LPNs
A Signature is now required for Name and Address Changes
To safeguard your pin number, signatures are necessary for both name and address changes.
Address Change - Submit your request for an address change in writing. Include your license number, name, and address change. Please PRINT clearly and include your signature.
Name Change - Submit your request for a name change in writing. Include your license number, previous name, and current name. Please PRINT clearly and sign the letter using your current signature.

LOST OR STOLEN NURSING LICENSE:
If you would like a duplicate nursing license, you may download the duplicate license request form that can be accessed from the Licensure Info/Forms tab. You will have to print the form, complete the form, have it notarized and return it to the Board office with a $15.00 check or money order.
