Verification of Licensure/Examination Scores

VERIFICATION OF LICENSURE

The Missouri Board of Healing Arts does not charge a fee to verify a license to another state or agency. If there is a form that needs to be completed, please fax the form to Donna Ellis at 573/751-3166 or mail it to P.O. Box 4, Jefferson City, MO 65102.

If no form needs to be completed but a letter is required, please submit the following information to Donna Ellis via email at donna.ellis@pr.mo.gov:

• The full name of the licensee;
• Profession;
• License number;
• Address that the letter needs to be sent.

EXAMINATION SCORES

The Missouri Board of Healing Arts charges a fee to provide examination scores to another State Board if you took the Missouri State Board examination (Not FLEX or USMLE). Please submit the fee along with the following information to the Board of Healing Arts, P.O. Box 4, Jefferson City, MO 65102:

• The full name of the licensee;
• Profession;
• License number;
• Approximate date of examination;
• Address that the scores need to be sent.

The fee for physician examination scores is $50.
The fee for physical therapist examination scores is $25.
