NOTICE - Revised Claim Process: Assignment and Release Procedure

Attached are revised instructions and the Next of Kin document from the Missouri Insurance Guaranty Associations. Complete instructions are included on their website (www.mo-iga.org). The funeral home assignment 1 pg is for example only. This will be the form that is completed and sent to the funeral home to be returned to receive payment. The forms have been simplified so that the new release forms are now a single page, one-sided document.

For Missouri claims that were submitted after October 23, the funeral homes will be receiving the MO Release Post 10-22-08 form. This is basically the same language as the "Universal" release except the restriction to the pre-Oct. 22 date has been removed. This form will be completed for each claim specifically and will be sent to the funeral homes PRIOR to their receiving funds. The funeral home will need to sign the form and return to the SDR. To clarify, the special deputy receiver has prepared the Mo Release Instructions post 10.22.08 claims.

There is very little difference between the two revisions. PLEASE NOTE: It is a requirement that funeral homes have the completed and signed Next of Kin form for all claims with a date of death occurring after Feb 1, 2009.
