AUTOMATED SECURITY ACCESS PROCESSING Forms

Automated Security Access Processing forms (ASAP) forms are being distributed for completion and to be submitted to us by fax so that your information can be entered into the electronic system to establish a user profile and assign your permanent ID and temporary password. You will receive your permanent ID by email about a week prior to the scheduled state training visit. Once your permanent ID and your assigned temporary password have been activated, you will be prompted to change your password. Please keep hard copy of the ASAP form until state training visit to your facility. At that time you must give the hard copy that includes your original signature to state staff for our files.
