Notice: Renewal for the State Board of Chiropractic Examiners
Attention Please note that the online renewal system will be down from 5 p.m. on February 17, 2017 through February 20, 2017 for planned system maintenance.
If your chiropractic license expires February 28, 2017 and you want to utilize the online renewal system, you must complete the renewal process prior to midnight February 28, 2017. The online renewal system will shut down at 11:59 p.m. February 28, 2017. Due to the potential volume of online renewals, licensees are encouraged to start the online renewal process no later than 11:45 p.m. February 28, 2017. The fee to renew the license is $50 until April 3, 2017 when the emergency rule expires. After April 3, 2017 the renewal fee will return to $125.
If mailing the renewal form, it must be postmarked no later than 2/28/17. If a renewal is postmarked after 2/28/17, the licensee will be sent information on reinstating the license. Along with the renewal fee, a $100 reinstatement fee is required.
When corresponding with the State Board of Chiropractic Examiners by e-mail and/or facsimile (fax), please refrain from including any sensitive personal identifying information such as Social Security Number or Date of Birth. The State Board does not need you to include sensitive personal information, in order to respond to your email or fax.
