If you would like to apply for a license to practice as an Athletic Trainer in the State of Missouri, the entire application can be downloaded by clicking here.

In order to provide access to our application forms from our website with the highest degree of compatibility, the applications are available in Adobe Acrobat format. To view the application forms, Adobe Acrobat Reader version 4.0 or later is required. If you do not have the reader or if you need a current version, it can be downloaded free of charge from Adobe by clicking the button at the bottom of this page. If you have a current version of the Adobe Acrobat Reader and still experience problems accessing the application, it is probably due to a slow Internet connection speed. To work around this problem, try to download the application directly to your computer. To do this, right click on the application link and click "save target as". This will allow you to save the application as a portable document file directly to the location of your choice on your computer. Once the file is downloaded, you can open it from that location.
